Client Authorization To Obtain Court Records

Client authorizes Clean Slate Docs, LLC, including its staff and authorized representatives, to request, obtain, and receive copies of court records and related documents necessary to prepare and file the Client’s expungement petition.

This includes, but is not limited to:

Certified docket entries, disposition records, and judgments
Charging documents
Probation/sentence completion confirmations
Case summaries from District or Circuit Court
Any publicly accessible records required for eligibility under applicable state law

If the case is in Maryland, this includes records permissible under Md. Crim. Proc. §10-105 and Maryland Rules 4-501–4-512.

2. Scope & Duration of Authorization

Client understands:

Clean Slate Docs is not a law firm, and this authorization does not create an attorney-client relationship.
Provider may communicate directly with court clerks, probation offices, and other public agencies solely to obtain records needed for the petition.
Authorization is valid for 90 days from signature unless revoked sooner in writing.
Client may revoke authorization at any time by written notice, except for actions already taken.

3. Limitation of Use

Provider may only use obtained records for:

Preparing the expungement petition
Filing documents with the court
Verifying eligibility

No other use is permitted.

4. Acknowledgement & Signature

By signing below, Client acknowledges that:

Client has read and agrees to all sections of this Agreement, including the Client Authorization to Obtain Court Records.
Client voluntarily grants this authorization.
One signature applies to all parts of this Agreement.
