Professional Services Agreement


This Professional Services Agreement and Authorization (“Agreement”) is entered into as of the date signed below, between Clean Slate Docs (“Provider”) and the individual signing below (“Client”). This Agreement governs the terms under which Provider will assist Client with expungement services in the applicable state listed below.

I. Purpose of This Agreement

This Agreement outlines the professional services provided to assist Client with the expungement of eligible records under applicable state law. Provider will guide the process from start to finish with accuracy, compliance, and clear communication.

II. Scope of Services

Provider will:

  • Review Client records to confirm eligibility for expungement
  • Prepare and file all required petitions with the appropriate court(s)
  • File electronically where applicable (including Maryland’s official e-filing system)

Important:
This Agreement does not include representation in hearings or appeals. If a court appearance is required, Provider may arrange attorney representation under a separate agreement at a fee of $450 per appearance.

III. Client Responsibilities

Client agrees to:

  • Provide accurate and complete information
  • Submit requested documents promptly (e.g., court records, dispositions)
  • Disclose prior expungement filings or related matters
  • Respond promptly to communications
  • Attend any required hearings

Failure to meet these responsibilities may result in delays or dismissal of the case.

IV. Fees and Payments

  • Service Fee: $798
  • Payment Terms:
    • 50% due at checkout
    • Remaining balance charged when petition is ready for filing
  • Rush fees (if selected) will be processed within 1 week

Full payment is required before filing with the court.

V. Confidentiality

All Client information will be handled with strict confidentiality and used solely for purposes related to the expungement process.

VI. Termination of Services

Either party may terminate this Agreement with written notice.

If terminated:

  • Client is responsible for work completed up to that point
  • Original documents will be returned within 10 business days

VII. Service Commitment & Guarantee

Provider guarantees:

  • Diligent, accurate, and professional handling of the case
  • Clear communication and timely updates
  • Compliance with all legal and procedural filing requirements

If a filing is rejected due to Provider error, it will be corrected and refiled at no additional cost.

A full refund will be issued if:

  • Client is determined not legally eligible during review
  • The court denies the case due to Provider error

Refunds will not be issued if denial results from:

  • Incomplete or inaccurate Client information
  • Failure to provide required documents
  • Failure to attend required hearings

Final eligibility determinations are made by the court.

VIII. Governing Law

This Agreement is governed by the laws of the state where the expungement case is filed.

IX. Authorization to File & Obtain Records

By signing this Agreement, Client expressly authorizes Clean Slate Docs to:

  1. Prepare and File Documents
  • Prepare, sign (where permitted), and file expungement petitions and related documents
  • Submit filings to courts and appropriate government agencies on Client’s behalf
  1. Obtain Court Records
  • Request, obtain, and receive court records necessary for eligibility verification and filing

This includes, but is not limited to:

  • Certified docket entries, dispositions, and judgments
  • Charging documents
  • Probation or sentence completion records
  • Case summaries from District or Circuit Courts
  • Publicly accessible records required under applicable state law

For Maryland cases, this includes records permissible under Md. Crim. Proc. §10-105 and Maryland Rules 4-501–4-512.

X. Scope, Limitations & Duration of Authorization

Client understands and agrees:

  • Clean Slate Docs is not a law firm, and this Agreement does not create an attorney-client relationship
  • Provider may communicate with court clerks, probation offices, and public agencies solely to obtain necessary records and process filings
  • Authorization is valid for 90 days from signature, unless revoked earlier in writing
  • Revocation does not affect actions already taken

Limitation of Use:
All records obtained will be used strictly for:

  • Eligibility verification
  • Petition preparation
  • Court filing

No other use is permitted.

XI. Acknowledgment and Agreement

By signing below, Client acknowledges that:

  • They have read and understand all sections of this Agreement
  • They agree to all terms, including the authorization provisions
  • They voluntarily grant all permissions stated herein

This is a single, unified Agreement. One signature applies to all sections above.
